When submitting your application for the Combined Internship and Masters Degree Program, include the following items:
- Completed and signed Supervise Practice Program Application found at http://depdpg.org/
- Statement of Intent describing your background, statement of purpose, and professional goals. This statement also suffices for your Graduate School application.
- Recommendations from three individuals in sealed and signed envelopes. You may also submit 3 Graduate School Personal Reference Forms at this time or you can send these with your Graduate School application. Recommendations for Graduate School should come from persons familiar with your academic potential.
- Declaration of Intent to Complete Degree and/or ADA-approved Minimum Academic Requirements signed by the Program Director. If you have already completed these, submit the Verification of Program Completion form signed by the Program Director.
- Transcript from each college or university attended to complete degree or ADA requirements. “Unofficial” transcripts (issued directly to you and not containing your college/university embossed seal) are acceptable for the internship application. However, you will need official transcripts to accompany your application to the Graduate School at OSU.
- Copy of Graduate Record Examination (GRE) Report of Scores. You should have the official report of these scores sent to the Admissions Office, The Ohio State University (code #1592).
- Application fee in the amount of $50.00. Make check payable to The Ohio State University.
